Welcome to Our Practice
We are grateful that you have chosen Lauderdale Eye Specialists for your visual needs. Our staff will do everything possible to provide you with the very best care.Our receptionist will kindly greet you as you arrive to our practice. Please inform the receptionist of your name and with which physician you have an appointment. You will be asked to fill out a brief Patient Registration Form and sign a Refraction Policy. For your convenience we have provided these forms on our website should you wish to complete them prior to your visit. Please bring in your insurance card(s) and photo identification.
Once checked in, one of our ophthalmic technicians will escort you to an exam room to collect your medical history and medication information and will complete a series of vision tests. These tests will assist the physician in diagnosing and treating your condition.
You will then meet with the physician for the remainder of your examination. Please feel free to address any concerns or questions relating to your vision. Our physicians are caring and attentive individuals who will make sure your concerns are addressed.
Upon completion of your examination, you will be led to the checkout desk where a receptionist will collect your co-pay and schedule your next appointment.
We strive to provide a comfortable and pleasant experience from the moment you walk in, until you leave our practice.
